In the event that a bus becomes unavailable for transportation to or from school, we will notify you as soon as possible to discuss other options to get students to and from campus. This form allows us to know which bus your student(s) ride so that we may contact you in a timely fashion.
At Middletown Middle School, students may accumulate community service hours in various ways during their 7th and 8th grade years. Students who have a minimum of 10 hours will be recognized at our 8th grade promotion ceremony. Down below, you'll find our Community Service form available to print. Students may use another form if they desire as long as it lists a description of what they did, how many hours, and signed off by an adult who supervised the hours (not a parent/guardian). Community Service is not required for middle school. We do recognize those students at 8th grade promotion with 10 or more community service hours.
In order for hours to count towards their middle school hours, we must have a completed form submitted to the office.
If you have any questions or concerns, please contact Sam Dean in the office. You can call (707) 987-4160, or email at firstname.lastname@example.org
